Births Deaths and Marriages

The Vital Statistics department provides assistance to the public in reference to birth, marriage, and deaths; dog licenses, clam licenses, business certificates, and genealogy research. The department records all city contracts; records declaration of trusts, and daily mail requests. We also provide notary services.

To Request Copies

Send a $12.00 check or money order made payable to the City of Quincy. Include the date requested and state whether it is a birth, death or marriage; the full name of person at birth, and enclose a self-addressed envelope.

Certified Long Form of Birth, Death or Marriage

Suitable for passport or legal purposes.

  $10.00 at City Hall Clerk's Office

  $12.00 by mail

Genealogy Search

Cannot be used for any legal purposes.

  $10.00 per hour (minimum fee $10.00)

  Plus record cost: $2.00 per copy

Please Note:  Births prior to 1925 must be typed and require an additional $10.00 Fee.

To Apply For A Marriage License

If you are being married in Massachusetts you may apply for a License in any city or town hall, both persons must be 18. Both persons must be present to apply (unless one person is in active military duty).

The fee is $45.00 to apply for a marriage license and there is a three day waiting period for the license to be ready. Couples may obtain a court waiver to waive the three days at district court, if necessary.
