The vital statistics department provides assistance to the public in reference to birth, marriage, and deaths; dog licenses, clam licenses, fishing and hunting licenses; business certificates, tax liens, ucc's and genealogy research. The department records all city contracts; records declaration of trusts, and daily mail requests. We also provide notary services.

To Request Copies:
Send a $12.00 check or money order made payable to the City of Quincy. Include the date requested and state whether it is a birth, death or marriage; the full name of person at birth, and enclose a self-addressed envelope.

Certified Long Form of Birth, Death or Marriage: $10.00 at City Hall Clerk's Office; $12.00 by mail.
Suitable for passport or legal purposes.
Certified Short Form of Birth Only: $3.00. Sutiable for sports or registry of motor vehicles.
Genealogy Search: $10.00 per hour (minimum fee $10.00) Plus record cost: $2.00 per copy. $10.00 Certified Copy. Cannot be used for any legal purposes.
Births prior to 1925: Must be typed: $10.00 Fee.

To Apply For A Marriage License:
If you are being married in Massachusetts you may apply for a License in any city or town hall, both persons must be 18. Both persons must be present to apply (unless one person is in active military duty).
The fee is $45.00 to apply for a marriage license and there is a three day waiting period for the license to be ready. Couples may obtain a court waiver to waive the three days at district court, if necessary.

1908 - Present
Filing a Business Certificate - $40.00 Valid for four years.
Renew Business Certificate- $40.00
Discontinuances - $5.00 fee
Certified copies of Business Certificates - $10.00
Hunting & Fishing Licenses - Available
Dog Licenses:
Dog license year begins April 1 - ends March 31.
Fee: $10.00 per dog
Valid RABIES CERTIFICATE must be shown at time of licensing.
